Health Information for Participation in Athletics

All athletes are required to have an annual physical examination by a health care provider to certify that the student is physically fit to participate in athletics in order to be eligible to practice or participate in athletics or any competitive sports. The Medical Examiner’s Statement provides documentation of this physical exam and is included in the Athletic Packet. The ENTIRE athletic packet must be completed and returned prior to practice or participation in athletics. These forms are provided for your convenience and may be downloaded, completed, and returned to your school Health Office.
