Student Health

A comprehensive physical examination is basic to understanding the health needs of students. All students enrolling in the San Mateo Union High School district are required to present a Medical Examiner's Statement soon after registration and not later than the end of the first semester. Athletes are required to present a Medical Examiner’s Statement annually with the complete athletic packet.

Links to Health Forms required for student enrollment and athletic participation are provided below for your convenience (with the exception of the Annual Medical Emergency Form which is inserted in the student/parent handbook).  Additional information regarding District immunization & TB requirements is also provided. Forms may be downloaded, completed, and returned to your school Health Office.
