Health Information for All Students

Health Forms required for student enrollment are provided for your convenience (with the exceptionof the Annual Medical Emergency Form which is inserted in the student/parent handbook).  The Student Health Inventory and Medical Examiner’s Statement are required for all students upon enrollment. The Emergency Care Plan and Medical Consent provide emergency instructions from health care providers for students with a health condition or concern (required annually).  The Medication Authorization is required for students who need to take medication during the school day (required annually). Additional information regarding District immunization & TB requirements is also provided.  Forms may be downloaded, completed, and returned to your school Health Office.  For those students wishing to participate in athletics, please refer to Health Information for Participation in Athletics.
