If you are requesting a copy of your transcript, please print and complete this "Transcript Request Form" Then send it along with a $5.00 money order (made payable to Peninsula High School) to:

Note: Mail in requests are responded to within two working days once request is received.
If you prefer to pick up your transcript in person, please print and complete the Transcript Request Form. Please call the Student Data Analyst, Alexandria Carrasco, at 650-558-2419 to make an appointment to pick up your request (Monday – Friday 8:00am to 3:00pm) or send an email to email@example.com. At the appointment please bring with you the completed form, your picture identification and $5.00 cash (checks not accepted). You can leave with your transcript that same day.
OUTSTANDING BOOK BILLS:
If you have an outstanding book bill with Peninsula High School or another transfer school, please pay the bill or return the outstanding book(s) to the site where the debt was incurred. Next, request that person at that site either e-mail (firstname.lastname@example.org) or call Peninsula High School 650-558-2402 to notify of the cleared debt.
