Immunization Requirements
Immunization Requirements for Students Attending School in the San Mateo Union High School District*
Effective July 1, 2019
In order to complete enrollment procedures and to attend classes, each student must have an immunization record from a health care provider on file in the Health Office documenting the dates (month/day/year) that the following vaccines were given:
- Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) -- 5 doses
- 4 doses OK if one was given on or after 4th birthday.
- 3 doses OK if one was given on or after 7th birthday.
- For 7-12 graders, at least 1 dose of pertussis-containing vaccine (Tdap) is required on or after 7th birthday
- Polio (OPV or IPV) -- 4 doses
- 3 doses OK if one was given on or after 4th birthday
- 3 doses OK if one was given on or after 4th birthday
- Hepatitis B --3 doses
- Measles, Mumps, and Rubella (MMR)--2 doses
- Both given on or after 1st birthday
- Varicella (Chickenpox)--2 doses
Students transferring to the District from another school in the U.S. are generally given 30 days for their prior school to transfer proof of immunization (vaccine catch-up schedule). If proof of immunizations is not received from the prior school within 30 days, the parent/guardian will be asked to provide proof of the above immunizations.
