Immunization Requirements

Immunization Requirements for Students Attending School in the San Mateo Union High School District* 

Effective July 1, 2019

In order to complete enrollment procedures and to attend classes, each student must have an immunization record from a health care provider on file in the Health Office documenting the dates (month/day/year) that the following vaccines were given: 

  • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) -- 5 doses
    • 4 doses OK if one was given on or after 4th birthday.
    • 3 doses OK if one was given on or after 7th birthday.
    • For 7-12 graders, at least 1 dose of pertussis-containing vaccine (Tdap) is required on or after 7th birthday
  • Polio (OPV or IPV) -- 4 doses
    • 3 doses OK if one was given on or after 4th birthday

  • Hepatitis B --3 doses

  • Measles, Mumps, and Rubella (MMR)--2 doses
    • Both given on or after 1st birthday
  • Varicella (Chickenpox)--2 doses

Students coming to SMUHSD from outside the state or country are required to provide proof of the above immunizations prior to starting class (vaccine catch-up schedule).

Students transferring to the District from another school in the U.S. are generally given 30 days for their prior school to transfer proof of immunization.  If proof of immunizations is not received from the prior school within 30 days, the parent/guardian will be asked to provide proof of the above immunizations.
